Final Reminder – You Will Be Dropped Tomorrow Without Vaccination Proof
Dear Student,
Our records show that you have not yet uploaded your proof of final vaccination against COVID-19, even though you are registered for one or more classes that are meeting on campus this fall. As you have been told in previous emails: You must be vaccinated against COVID-19 in order to attend on-campus classes.
You will be dropped from all on-campus classes in which you are registered unless you submit proof of your final vaccination by the end of day tomorrow – Friday, Oct. 1. (Your final vaccination is your second shot for the Pfizer or Moderna vaccine, or your first shot if it's Johnson & Johnson.)
In addition, you must comply with these requirements until two weeks after your final shot:
- Obtain a lab test for COVID-19 (home tests will not be accepted) – and upload your negative test result to the PyraMED records system – within three days before each visit to campus
- Wear a face mask at all times, indoors and outside, while on campus
- Practice social distancing at all times on campus
If you believe you have already uploaded your vaccination record to the PyraMED system, you can verify the upload by following these steps:
- Log in to the PyraMED system
- Click on the My Profile tab, then select "Immun. History" and click to download your record
If you do not see your final vaccination listed on that record, send an email to dahealthclerk@deanza.edu and ask them to check your records. Be sure to include your full name and CWID, and the date that you received this reminder email.
All students, including those who are fully vaccinated, must wear a face mask indoors and use the Optimum HQ check-in service each time they come to campus.
